A summary panel is useful here (on each page).

We did this for Kidly (one of the examples in that post).




A detailed enough summary panel for me not to worry about data entry issues is pretty much identical to the original form itself, so at this point one may as well put the whole form on one page.

As long as you...

1. Don't break back button. 2. Provide an overview so user can jump around. 3. Give a progress bar.
